Position: Wraparound Facilitator

Location: Any HPMHC office location (Hays Norton Phillipsburg Colby Goodland Osborne)

Pay: $22.36 per hour $500 SIGN-ON BONUS!

Job Type: Full-Time In-Person

Schedule: typical hours Monday- Friday 8am-5pm

Responsibilities Include:

• Facilitating Wraparound planning meetings with families and support teams
• Helping families navigate the SED/HCBS Waiver and PRTF Diversion Grant programs
• Coordinating services and community resources to support treatment goals
• Developing individualized plans that focus on strengths needs and long-term success
• Collaborating with families educators providers and community partners
• Monitoring progress and adjusting plans to ensure positive outcomes
• Advocating for children and families within schools agencies and community systems
• Completing documentation and maintaining compliance standards
• Traveling within the service area to meet with families and community partners

• Experience counts! We accept equivalent work experience in human services (teaching daycare CNA etc.) or a BA/BS degree. One year of related work one year of college.
• Great with peoplestrong communication and empathy are a must. Experience working in human services behavioral health social services education or a related field preferred.
• Comfortable in a variety of human service settings. Must be able to work effectively with children experiencing Severe Emotional Disturbance (SED) and their families.
• Able to pass drug screen and required background checks.
• Must have valid drivers license.
