The Part-Time Wellness Nurse is a non-clinical supportive services professional who works in collaboration with the full-time Service Coordinator(s) at the Essex Co-op. The Wellness Nurse does not provide direct medical care diagnosis or treatment and does not duplicate or replace services already provided by healthcare Part-Time Wellness Nurse functions within a supportive educational and referral-based capacity ensuring compliance with HUD regulations while enhancing resident access to wellness-related resources and services. This position reports to the Civic Works Regional Service Coordinator Manager.

CSI SUPPORT AND DEVELOPMENT
CSI Support & Development Services is a resident/member-controlled organization that utilizes a cooperative management system and engages its resident membership in decision-making at every level of its operations. Since 1965 CSI have operated as a mission-driven non-profit dedicated to providing the highest quality affordable housing communities for seniors. The CSI Housing Foundation is committed to regularly evaluating and addressing the evolving needs of its residents through innovative programs and services. CSIs vision is to preserve and grow cooperative senior housing communities helping members lead full healthy social lives.
KEY RESPONSIBILITIES
- Coordinate with the Resident Service Coordinator to engage with current residents and be introduced to any new resident upon move-in.
- Ensure the delivery of face-to-face interaction with the residents to understand their supportive service and health needs.
- Assess residents ADL/IADL status while focusing on their functional limitations and chronic health needs.
- Serve as a liaison with primary care physicians hospitals or other health care providers as necessary and appropriate to assist with self-care disease management medication self-management or coordinating care between different care providers.
- With support from the RSC collaborate with Area Agencies on Aging (AAA)/Aging and Disability Resource Center (ADRC) to ensure that necessary community-based supports are accessible and available to residents.
- Check vital signs of the residents-blood glucose blood pressure pulse rate etc.
- Coordinate on-site health screening and vaccinations-diabetes hypertension flu shots; COVID-19 vaccinations.
- Develop and oversee preventive health programs that are focused on the optimizing ADL/IADL functions through exercise diet and other preventive health activities; teach medications self-management and self-care skills.
- Implement programs that focus on chronic disease management (COPD Hypertension and Disease)
- Implement programs that address common health and wellbeing concerns of older adults such as but not limited to the following: fall prevention nutrition for older adults. Grief and loss incontinence healthy weight loss brain health changing sleep patterns etc.
- Health education (dental hygiene hospice brain health hearing resources smoking cessation food safety healthy heart understanding CPR medication management diabetes management vision care understanding Medicare managing life emotions navigating Social Security services).
- Assist with medication self-management in accordance with state law. Including assessment of individual residents to determine ability for self-administration.
- Monitor transitions home following emergency department visits or discharge from hospital/rehabilitation center.
- Collaborate with the Resident Service Coordinator to address resident compliance issues related to health concerns and network with community partners.
Preferred Skills
- Experience in community health nursing including such practices as conducting wellnessand prevention activities providing health education and coaching consideringcommunity/population needs and engaging with community service providers.
- Experience in working with or providing care to older adults.
- Experience conducting assessments that accurately and appropriately identify anindividuals physical and mental health and functional needs and using this information todevelop and implement care plans.
- Experience using motivational interviewing active listening and person-centered careconcepts to help encourage individuals to engage in addressing and managing their ownhealth care concerns.
- Knowledge of or certification in delivering standardized or evidenced-based healtheducation and prevention programs (e.g. Chronic Disease Self-Management Program).
- Cultural competency in working with diverse populations and individuals from differentethnic and racial backgrounds including working with non-English speaking adults.
- Familiarity and understanding of working with individuals with mental health needs.
- Knowledge of primary care practice operations.
- Strong verbal written and interpersonal communication skills.
- Understanding of using data to monitor and improve program practices and operations.
Qualifications
QUALIFICATIONS
- Registered Nurse (RN) in the State of Maryland. A Licensed Practical Nurse (LPN) maybe considered with appropriate skills and experience.
- Maintains a current unencumbered state license as a professional Registered Nurse (RN)
- Minimum of one (3) year experience in home health assisted living hospital or long-term care environment.
- Demonstrates knowledge of nursing practices techniques and methods applied to health and wellness resident care and federal state and local regulations.
- Demonstrates knowledge of good assessment skills.
- Ability to handle multiple priorities.
- Possesses written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care.
- Competent in organizational and time management skills.
- Demonstrates good judgment problem solving and decision-making skills.
- Demonstrates proficiency in computer skills Microsoft Office (Windows Outlook Excel) and Sunrise applications with the ability to learn new applications.
- Ability to work either 8am-1pm or 9am to 2pm.
- Ability to translate medical jargon into easily understandable lay language and tocommunicate at appropriate literacy and cognitive levels.
- Aptitude in providing education on health and functional conditions commonlyexperienced by older adults.
- Understanding of and sensitivity to the needs of low-income older adults and personswith disabilities.
- Ability to self-direct and work independently but also function well as part of a team.
- Ability to multi-task and establish priorities in a sometimes busy and stressfulenvironment.
- Ability to establish positive and collaborative relationships with residents staff andcommunity-based providers.
- Basic computer proficiency including ability to maintain data in an electronic databasecommunicate via e-mail use internet and operate word processing software to createprogram materials.
ADDITIONAL REQUIREMENTS
- Valid drivers license. No travel required.
- Successful completion of a criminal background check and drug screening.
- Proficiency in Microsoft Office case management databases and reporting systems.
- Strong interpersonal communication and documentation skills.
