We seek a highly skilled and experienced Registered Nurse with specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room. The Registered Nurse will be responsible for providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily tasks include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Critical to this role is strong communication and strong collaboration with patients providers and other members of our team. At least 2 years of experience and exceptional skill is required.
Responsibilities:
Conduct patient assessments and develop individualized treatment plans.
Administer medications and monitor patient responses.
Provide skilled nursing care in various specialties including Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU and Operating Room.
Manage hospice care for patients with advancing age frailty and serious illness.
Collaborate effectively with interdisciplinary teams including physicians therapists and social workers.
Maintain accurate and timely documentation of patient care and treatment plans.
Communicate clearly with patients and their families regarding care plans and progress.
Required Skills & Certifications:
Active Registered Nurse license in the state of employment.
Bachelors degree in Nursing or related field.
Minimum of two years experience in hospital home health or hospice care.
Clinical assessment medication administration and treatment planning skills.
Strong interdisciplinary team collaboration abilities.
Excellent communication and interpersonal skills.
Drivers license car insurance and reliable automobile for field work.
Preferred Skills & Certifications:
Experience in pediatric care particularly with feeding and swallowing disorders.
Specialized certifications related to Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room nursing.
Special Considerations:
Ability to work in fast-paced and potentially high-stress environments.
Strong commitment to patient-centered care and ethical nursing practices.
Must be able to collaborate closely with patients families and healthcare team members.
No weekend or holiday shifts required.
Scheduling:
Standard weekday schedule with no weekends or holidays.
Shift assignments determined based on patient needs and team coordination.
