Responsibilities

The Social Services Supervisor is responsible for the daily functions of the Social Services department. The Supervisor is an active participant in the facility interdisciplinary team (IDT) attending care plan conferences IDT meetings and other committee meetings. Responsible for providing psychosocial-emotional support and services to residents and families. Assists residents and family members in understanding coping and adapting to the personal emotional and psychosocial issues associated with the residents illness disability or recovery.

Qualifications

Education

o Bachelors Degree in Social Work or Human Services required

o Masters Degree in Social Work (MSW) preferred

Experience

o 3 years of relevant experience in the healthcare industry or healthcare setting required

o 1 year of experience in a supervisory role preferred

o Experience with PointClickCare or EHR systems (preferably in a Skilled Nursing facility/setting) preferred

Licenses and Certifications

o LCSW - State of California Licensed Clinical Social Worker preferred
