REQUIRED ATTACHMENTS:1. Attach current valid California Teaching Credential OR 30-Day Sub Permit.2. Attach TB skin test certificate read or risk assessment within the last 4 years3. Attach fingerprint clearance receipt from the San Diego County Office of Education (SDCOE) Clearinghouse. This is different than the Certificate of Clearance you completed for your credential No other Fingerprint clearance will be accepted. (If you are already in the SDCOE Clearinghouse system and do not have a receipt please note that on application and it will be verified with the SDCOE.)4. AB2534 (mandatory Meployemnt Disclosure) section in application must be complete.
