Schedule: MondayFriday 8:00 a.m.4:00 p.m.

Were looking for a compassionate skilled Speech-Language Pathologist to join our team and provide individualized care to patients throughout our service area. Youll help patients improve communication swallowing cognition safety and quality of life while supporting independence at home.

1. Evaluate patients and develop individualized plans of care.
2. Provide evidence-based speech language cognitive and swallowing therapy.
3. Monitor progress and adjust treatment plans as needed.
4. Educate patients and caregivers on treatment strategies and home programs.
5. Complete timely and accurate clinical documentation.
6. Collaborate with physicians and the interdisciplinary care team.
7. Participate in care conferences quality initiatives and educational programs.
8. Travel safely throughout the Granite VNA service area to provide care in patients homes.

1. Current Speech-Language Pathologist license in New Hampshire or eligibility for licensure.
2. Strong clinical communication and organizational skills.
3. Ability to work independently while collaborating effectively with a multidisciplinary team.
4. Commitment to patient-centered evidence-based care.
5. Valid drivers license and reliable transportation.

1. Masters degree in speech-language pathology communications sciences and disorders or communications disorders.
2. Graduate of an approved speech-language program (ASHA) with a certificate of clinical competency in speech-language pathology (CCC-SLP) accompanied by current New Hampshire speech-language pathology license.
3. Two years of inpatient/skilled/acute or community health speech-language experience.
4. Valid drivers license reliable transportation and an acceptable driving record.
