RN Case Manager (Part-Time to Full-Time 3240 hrs/week)

Your Role
As a key member of our interdisciplinary care team you will work under the supervision of a Clinical Manager to coordinate and deliver high-quality patient-centered care. Youll play a vital role in ensuring comfort dignity and quality of life for those we serve.
Key Responsibilities
- Perform ongoing comprehensive assessments of patients and families
- Develop and oversee individualized care plans in collaboration with the care team
- Lead and participate in interdisciplinary case conferences
- Coordinate care with physicians hospice staff volunteers and community partners
- Advocate for patients and families addressing clinical emotional and psychosocial needs
What Youll Bring
- Active Massachusetts RN license in good standing
- Associates degree in Nursing (BSN preferred)
- Experience providing compassionate care in home or facility-based settings
- Strong interpersonal and communication skills
- Ability to work independently while thriving in a team environment
- Sound clinical judgment flexibility and strong organizational skills
- Valid drivers license and access to a reliable insured vehicle

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education training relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post. Care Dimensions also offers a comprehensive benefits package including employer-sponsored health insurance paid time-off tuition reimbursement student loan paydown supplemental insurance options and more which adds to the companys generous total rewards package.
Pay Range
$50 - $58 USD
Benefits are offered to employees that are scheduled to work 20 hours/week which include a generous earned time (vacation days) program tuition reimbursement scholarship programs student loan paydown program two retirement plans in addition to medical/dental/vision/life/disability insurance and so much more!
