Your Role

As a key member of our interdisciplinary care team you will work under the supervision of a Clinical Manager to coordinate and deliver high-quality patient-centered care. Youll play a vital role in ensuring comfort dignity and quality of life for those we serve.

Key Responsibilities

• Perform ongoing comprehensive assessments of patients and families
• Develop and oversee individualized care plans in collaboration with the care team
• Lead and participate in interdisciplinary case conferences
• Coordinate care with physicians hospice staff volunteers and community partners
• Advocate for patients and families addressing clinical emotional and psychosocial needs

What Youll Bring

• Active Massachusetts RN license in good standing
• Associates degree in Nursing (BSN preferred)
• Experience providing compassionate care in home or facility-based settings
• Strong interpersonal and communication skills
• Ability to work independently while thriving in a team environment
• Sound clinical judgment flexibility and strong organizational skills
• Valid drivers license and access to a reliable insured vehicle
