Resource Navigator

SUMMARYThe Resource Navigator works as part of an intensive residential treatment team that provides care treatment and rehabilitation for persons served who are experiencing significant mental health/psychiatric symptoms; who are demonstrating significant functional impairments; and who may have co-occurring JOB FUNCTIONSTo perform this job successfully an individual must be able to satisfactorily perform each essential function listed below. Crisis Assessment and medical/behavioral persons served in obtaining all financial medical and community benefits for which they are current directory of a variety of community in discharge planning and care coordination process of the person Clinical Director in obtaining prior authorizations from insurance rent from persons served or rep payee at the beginning of each client invoices for Room and an updated waitlist of referrals to facility take and return phone calls about facilitys admissions process in a timely Progress notes and and Individual Counseling the treatment/rehabilitative activities and in weekly staff significant / major incidents or high levels of mutual trust and support with coaching/learning good performance and promotes employee and promotes consumer choice and facilitate to perform duties independently and as a the operation in a positive manner to the community and outside persons served community integration and choice accepts responsibility for attendance and self in professional and ethical and is willing to assist others accepts productive individual and teamwork information in a confidential in an open and honest proper authorities of circumstances or situations that present possible health and implements established policies on health safety security code of conduct regulation and company tasks as RESPONSIBILITIESNoneMinimum Knowledge and Skills required for the JobThe requirements listed below are representative of the knowledge skill and/or abilities required to perform the job.
