Job Summary:

Join the team at Oak Park Oasis a Joint Commission Approved facility dedicated to providing comprehensive medical services and care for our patients in Oak Park Illinois. As a Registered Nurse (RN) you will play a vital role in our care team working closely with our multidisciplinary healthcare professionals to deliver high-quality patient-centered care.

Responsibilities:

• Provide individualized patient care and assessment to patients with diverse needs including those with complex medical conditions

• Develop and implement patient care plans in collaboration with the care team physician and family

• Monitor patient progress update care plans and communicate with the care team and family about patient status

• Coordinate with various healthcare professionals and community resources to ensure continuity of care and seamless transitions

• Participate in staff development and education staying current with industry trends and best practices

• Collaborate with our medical director to develop and implement evidence-based care practices
