Registered Nurse (RN) – Hospice Field Case Manager | East Pasco

Gulfside Healthcare Services is seeking a compassionate Registered Nurse (RN) to provide hospice field care throughout East Pasco. Our Hospice Field RNs support patients and families in private residences assisted living facilities and skilled nursing facilities while providing comfort-focused care education and support during the end-of-life journey.

The Hospice Field RN manages an assigned patient caseload develops individualized care plans collaborates with our interdisciplinary team and provides guidance to LPNs CNAs and other members of the care team.

EDUCATION AND QUALIFICATIONS:

Active Florida Registered Nurse (RN) license or Compact State RN license required
Minimum of one (1) year nursing experience preferred
Hospice home health case management or acute care experience preferred
Strong assessment communication critical thinking and time management skills
Compassionate organized and able to work independently while supporting a team-based approach to care
Valid Florida drivers license reliable transportation and proof of automobile insurance required
Proficiency with Electronic Medical Records (EMR) preferred
