Registered Nurse Every Weekend

Weekends - Saturday & Sunday 8-10 visits per day

We are looking for an experienced Registered Nurse (RN) to join our home health this role you will oversee patient care support caregivers ensure quality care and help coordinate services for patients in their homes.

• Visit patients in their homes and monitor the quality of care.
• Assess patient needs and help develop appropriate plans of care.
• Support and guide caregivers and clinical staff.
• Communicate with patients families physicians and the care team.
• Ensure all documentation is accurate complete and submitted on time.
• Identify and resolve patient or staff concerns.
• Follow company policies and state and federal regulations.
• Use sound nursing judgment to solve problems and improve patient outcomes.

• Adults

• Active Indiana Registered Nurse (RN) license.
• At least 2 years of home health nursing experience (required).
• Valid drivers license and reliable transportation.
• Strong assessment communication and organizational skills.
• Experience using electronic medical records (EMR/EHR).
