Job Summary

We offer a wide range of services includingmental health care residential and outpatient substance use treatment and primary health we provide transitional support for individuals re-entering the community after involvementin the criminal justice system. By integrating physical and behavioral health we empower individuals toovercome challenges by addressing social determinants of health fostering resilience and 214 Mens Residential Step Down (MRSD) is a program of HealthRIGHT 360 located at Alamo Squareneighborhood in San Francisco. It is a 64-bed male transitional housing for individuals that have completedat a minimum of 30 days residential substance use disorder treatment. MRSD provides temporary housingto assist our clients in their next stage of On-Cal Recovery Coach is responsible for assessing client needs and strengths in support of healthand recovery. Through individual coaching with program specific requirements and case managementOn-Call Recovery Coaches assists clients in navigating systems of care while providing a supportiverecovery environment. On-Call Recovery Coaches are responsible for maintaining and assessing the safetyand security of the program and its residents through continued house runs while providing a supportiveenvironment. Through effective communication On-Call Recovery Coaches relay relevant information toother members of the interdisciplinary team and perform crisis intervention as needed. On-Call RecoveryCoaches ensure that the facility environment is safe and RESPONSIBILITIESIndividual Responsibilities: Provides direct services such as intakes assessment crisis intervention and case management asneeded. Responsible for retention and completion of clients facilitates individual coaching sessions witheach caseload client. Provides learning experience opportunities and offers support to assist clients in meeting theirlong-term recovery goals. Proactively links clients to both internal and external resources based on their recovery needs andfollows up on the progress/status. Performs house runs to ensure and maintain accountability of all clients and the safety andsecurity of the facility. Performs crisis intervention and communicates with treatment team as unforeseen situationsarise. The Recovery Coaches documents client updates and incidents in the facility log daily performsCPR and First Aid as needed and works weekends and holidays as Responsibilities: Facilitates community meetings and ensures compliance with facility rules. Performs crisis intervention and communicates with team as unforeseen situations arise. The Recovery Coach will document all client updates and facility incidents daily in the facility log. Performs periodic house runs to ensure and maintain the safety and security of the facility. When assigned the DOT documents and accurately distributes client money ensures clientmedications are securely stored and properly accounted for and holds facility keys and floatercellular phones. As needed accompanies clients to offsite appointments related to their recovery. Attends and completes all required trainings and meetings as assigned by facility manager. Plans and facilitates client celebrations and special events. Recovery Coaches must be available to work weekends and holidays as Responsibilities: Collaborates with each client and other available internal and external resources to developtransition plans progress notes and appropriate updates in support of the health and recoveryneeds of the client. Properly documents all individual encounters and completes the discharge paperwork/processand required agency assessments in timely manner. Also maintains accurate records by data entering documentation into various electronic systemsfor all clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal andexternal evaluating perform other duties as Credentials and Experience High School Diploma or equivalent bachelors degree preferred. Drug and Alcohol registration or certification recognized by Department of Health Care Services(DHCS) preferred. First Aid and CPR Certified within 30 days of employment. Valid California Drivers License and automobile insurance. Experience working successfully with diverse populations specifically with issues of substanceabuse mental health criminal background and other potential barriers to economic self and Skills Knowledge of gender-responsive trauma informed and co-occurring treatment. Strong computer skills especially with Microsoft Office applications and internet applications. Ability to work within a frequently changing project scope while maintaining overall direction andstructured priorities. Self-motivated dependable creative and proactive approach to work; understands importanceof working independently and within a team environment. Ability to enter data into various electronic systems while maintaining integrity and accuracy ofdata. Excellent verbal and written communication skills and interpersonal skills. Ability to handle sensitive information in a confidential manner. Bilingual in the SF county language threshold desired.
