Job Summary

We are seeking a skilled Physical Therapy Assistant (PTA) to provide high-quality patient-centered physical therapy and rehabilitation services under the supervision of a Registered Physical Therapist (RPT).

This position is based in a Skilled Nursing Facility (SNF) and sub-acute inpatient rehabilitation setting providing therapeutic care to geriatric and post-operative patients. The PTA will implement established treatment plans support functional mobility and recovery monitor patient progress and maintain accurate clinical documentation.

Key Responsibilities
  • Provide physical therapy treatments and therapeutic interventions according to the plan of care established by the supervising Physical Therapist.
  • Assist patients with gait training therapeutic exercises neuromuscular re-education transfers and functional mobility.
  • Apply appropriate therapeutic modalities including heat cold ultrasound and electrical stimulation as directed by the treatment plan.
  • Monitor patient responses to treatment and identify changes in functional or clinical status.
  • Document treatments patient progress responses and outcomes accurately in the NetHealth (Rehab Optima) EMR system.
  • Communicate patient progress concerns and significant changes to the supervising Physical Therapist.
  • Educate patients and caregivers on therapeutic exercises transfers mobility techniques self-care and home safety.
  • Maintain compliance with facility policies clinical standards and applicable professional requirements.
Mandatory Requirements
  • License: Active and unrestricted California Physical Therapist Assistant (PTA) license.
  • Experience: 1-2 years of recent clinical experience as a Physical Therapy Assistant.
  • Certification: Current Basic Life Support (BLS/BCLS) certification.
Preferred Qualifications
  • Experience with NetHealth (Rehab Optima) documentation software.
  • Previous experience in a Skilled Nursing Facility (SNF) sub-acute rehabilitation inpatient rehabilitation or geriatric care setting.
  • Strong communication patient-care documentation and clinical skills.
