We are hiring a Physical Therapist for a 13-week contract in Napa CA. This role is in a Skilled Nursing Facility setting and is a great opportunity for a PT with SNF or short-term rehabilitation experience who is focused on helping patients improve mobility strength function and independence.
Responsibilities
Evaluate patients and develop individualized physical therapy treatment plans
Provide therapy services in a skilled nursing and rehabilitation setting
Support patients with mobility balance strength transfers and functional recovery
Monitor patient progress and adjust treatment plans as needed
Educate patients and caregivers on exercises safety and mobility techniques
Collaborate with nursing staff therapy teams and interdisciplinary care members
Complete accurate and timely documentation per facility standards
Requirements
Active California Physical Therapist license required
Minimum 1 year of Physical Therapy experience required
SNF or short-term rehabilitation experience preferred
BLS/BCLS certification required
Ability to work 40 hours per week
Flexibility to work Monday Friday or alternate schedule as needed
Must be open to possible float between nearby facilities
