Responsibilities -

Perform comprehensive in-home patient evaluation and develop tailored PT
Deliver therapeutic exercises manual therapy and functional mobility training
Coordinate interdisciplinary care with nurses OTs social workers and physicians
Educate patients and caregivers on a safe home exercise program
Document all patient interactions progress notes and plan updates in the EMR promptly
Maintain HIPAA compliance and uphold safety and sanitation standards on every visit

Required Qualification -

Active California Physical Therapist license (Doctorate or Masters preferred)
Min 1 year of Clinical PT experience within Home health is required
Current CPR/BLS certification
Reliable transportation with a valid CA drivers license and auto insurance
Strong clinical reasoning patient education and documentation skills
Ability to work independently and manage a travel intensive caseload effectively
