- Evaluate and develop individualized treatment plans
- Provide evidence-based physical therapy services to adult and geriatric patients
- Help patients regain strength mobility balance and independence
- Collaborate with PTAs OTs OTAs SLPs nursing and physicians
- Complete timely documentation in accordance with Medicare and regulatory guidelines
- Participate in interdisciplinary care planning and discharge planning
- Deliver outstanding patient-centered care while achieving positive clinical outcomes
- Graduate of an accredited Physical Therapy program
- Current Michigan Physical Therapist license (or eligible for licensure)
- Strong communication and clinical skills
- Experience in Skilled Nursing Post-Acute Care Inpatient Rehab Home Health or Outpatient is preferred but not required
