Schedule: 40 hours/week
Contract: 13 weeks
Guaranteed Hours: 40 hours/week

About the Role
We are hiring an experienced Physical Therapist for a 13-week contract in Petaluma CA. This role is in a Skilled Nursing Facility setting and is a great opportunity for a PT who enjoys helping patients improve mobility restore function and regain independence.
Responsibilities
- Evaluate patients physical abilities and develop personalized treatment plans
- Provide therapy to improve strength mobility balance and functional ability
- Educate patients and families on exercises safe movement and home strategies
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with nurses physicians therapists and interdisciplinary team members
- Maintain accurate and timely documentation of assessments progress and treatment plans
- Ensure care plans follow facility standards and therapy best practices
Requirements
- Active California Physical Therapist license required
- Minimum 1 year of Physical Therapy experience required
- SNF or similar healthcare setting experience preferred
- Bachelors Masters or Doctoral degree in Physical Therapy required
- Knowledge of physical therapy techniques modalities and adaptive equipment
- PointClickCare experience preferred
- Strong communication documentation and patient care skills
