Physical Therapist (Full Time) – Assisted Living Setting

We are currently seeking a motivated and experienced full-time Physical Therapist to join our team at a partnered Assisted Living Facility on the Upper East Side (NYC). All levels of experience are encouraged to apply and will train the right new graduate.

Responsibilities:

• Provide 1:1 physical therapy treatment to residents within the facility
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care in accordance with professional standards and the established plan of care
• Lead a weekly group exercise class focused on mobility and functional wellness
• Collaborate with facility staff and caregivers to support patient outcomes
• Plan and implement therapy programs to improve or restore functional abilities reduce limitations and enhance quality of life
• Perform and document initial evaluations progress notes and discharge summaries in a timely and accurate manner
• Monitor patient progress and modify treatment plans as needed to achieve optimal results
• Provide care with a strong understanding of geriatric and memory care populations

Qualifications :

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.

Additional Information :

The anticipated base salary range for this position is $90000.00-$105000.00. Salary is based on various factors including relevant experience knowledge skills other job-related qualifications and geography. Medical dental vision 401(k) paid time off and other benefits are also available subject to the terms of the Companys plan.
