All Heart Home Care is seeking compassionate and skilled Physical Therapists (Full-Time and Part-Time) to join our growing home health team in the Columbia Tennessee area. This role provides the opportunity to deliver personalized one-on-one care to patients in the comfort of their homes while enjoying a flexible schedule and a supportive work environment.

• Perform in-home evaluations to assess patient needs and develop individualized treatment plans
• Provide skilled physical therapy services to improve mobility strength balance and overall function
• Monitor patient progress and modify treatment plans as appropriate
• Educate patients and caregivers on home exercise programs safety measures and injury prevention
• Complete accurate and timely clinical documentation in accordance with company policies and state regulations
• Collaborate with physicians nurses case managers and other healthcare professionals to ensure coordinated patient care
• Travel to patient homes within the assigned service area

• Doctor of Physical Therapy (DPT) or equivalent from an accredited program
• Active Tennessee Physical Therapist license
• Current CPR certification
• Valid drivers license reliable transportation and current automobile insurance
• Strong clinical assessment and communication skills
• Ability to work independently and manage time effectively
• Previous home health experience preferred but not required
