About the Role
We are seeking a Physical Therapist for a 13-week assignment in a skilled nursing and long-term care setting in Bangs Texas. This role is ideal for a therapy professional who can evaluate residents create effective treatment plans and support improved mobility strength and functional independence.
Responsibilities

Evaluate patients and develop individualized therapy plans

Provide skilled physical therapy services in a SNF/LTC setting

Support mobility balance strength and functional recovery goals

Monitor patient progress and adjust treatment as appropriate

Document treatment sessions and patient response accurately

Collaborate with the care team to support positive outcomes
Requirements

Active Texas Physical Therapist license required

Minimum 1 year of Physical Therapist experience required

Skilled Nursing Facility or Long-Term Care experience preferred

Strong clinical assessment and documentation skills

Ability to work independently and as part of a therapy team
