Full Time- 8hrs- Days/Afternoons
The Pharmacist in our Transitions of Care unit at Cedars-Sinai Medical Center in Los Angeles California are critical to ensuring safe transitional care as patients are admitted and discharged. Our TOC pharmacists support institutional goals such as improvement in clinical care and reduce readmissions for CMS superbundle and population health patients. They also ensure regulatory compliance with California regulation (CA Business & Professions code 4118.5) by performing medication history reviews at admission and discharge for high risk patients.
