The Oncology Pathway supports patients throughout the cancer care continuum by coordinating services across diagnosis treatment and follow-up care. The department works collaboratively with multidisciplinary clinical teams to ensuretimelyaccess to care evidence-based treatment planning and a patient-centered experience that promotes quality outcomes and continuity of care.
Under the direction of the Oncology Executive Director the Oncology Social Workeris responsible forproviding psychosocial services in collaboration with patients caregivers multidisciplinary team membersand professional providers for Baptist Health cancer patients and their families.
Description:
Conduct comprehensive psychosocial assessments and develop patient- and family-centered care plans for oncology patients.
Provide evidence-based counseling coping strategies and emotional support while helping patients and families navigate the healthcare system.
Coordinate family meetings advocate for patient needs and connect patients with communitysurvivor and support resources.
Develop andfacilitatesupport groups while maintaining partnerships with community and national organizations.
Collaborate with the interdisciplinary oncology team to communicate care plans support discharge planning and improve patient outcomes.
Assistwith educational initiatives for oncology staff patients and families.
Adhere to Baptist Health policies infection prevention standards safety protocols emergency preparedness and all required training.
Requirements:
Bachelors degree in Social Work(BSW);Masters degree in Social Work(MSW) preferred.
Minimum of two (2) years of related social work experience.
Current Kentucky Clinical Social Worker license.
Must obtain Basic Life Support (BLS) certification through the American Heart Association within six (6) months of hire.
