About the Role

We are seeking a compassionate and reliable Home Health & Hospice Licensed Clinical Social Worker (LCSW) to support patients and families served by Select Home Health and this role you will provide professional medical social services in the home addressing social emotional and psychosocial needs while supporting patient dignity comfort and quality of life in accordance with the plan of care.

Responsibilities

• Provide medical social services to hospice patients and families in the home setting.

• Assess and address social emotional and psychosocial needs related to illness and end-of-life care.

• Collaborate with the interdisciplinary team to develop and support the patients plan of care.

• Counsel patients and families to support coping adjustment and decision-making.

• Communicate changes in patient condition to the supervisor and physician as needed.

• Connect patients and families with appropriate community and social service resources.

• Participate in care coordination discharge planning and team meetings.

• Supervise Bachelors-level social workers as applicable.

• Masters or Doctoral Degree in Social Work from a CSWE-accredited program (required).

• Current Licensed Clinical Social Worker license in the state of practice (required).

• Active NPI (required).

• At least (2)years of healthcare social work experience hospice or home health (preferred).

• Strong communication organization and interpersonal skills.
