Sequoia Transitional Care a beautiful and welcoming 99-bed skilled nursing facility is hiring a dedicated LVN Charge Nurse for our Part-Time shift. Whether youre experienced or newly licensed we offer a supportive environment training and growth opportunities to help you succeed.

• Provide direct nursing care to residents in accordance with individualized care plans

• Serve as the Charge Nurse during NOC shift overseeing CNAs and ensuring high-quality resident care

• Administer medications and treatments as prescribed

• Monitor residents for changes in condition and report to RN Supervisor or physician

• Document all care provided accurately and timely in EMR system

• Communicate effectively with residents families and interdisciplinary team

• Maintain compliance with safety infection control and facility policies

• Respond to emergencies and follow established protocols

• Participate in shift handoffs training and care planning

• Current California LVN license in good standing

• CPR certification

• Experience in a SNF or post-acute setting preferred but not required

• New graduates encouraged to apply we will train you!

• Strong written and verbal communication skills

• Computer literate and comfortable with EMR systems

• Compassionate dependable and a team player

• Must be able to provide recent medical clearance to work in SNF

• Must pass background check and be eligible to work in the U.S.

• Starting rate $30
