LPN, Licensed Practical Nurse Facility Based Nursing

Cooperative Home Care is hiring LPNs to join our nursing team!

Are you a compassionate and dependable nurse looking for flexible work in a nursing facility setting Cooperative Home Care is looking for dedicated LPNs to provide quality patient will work alongside other healthcare professionals in a supportive nursing facility environment while providing hands-on care to residents.

SCHEDULE: Weekends; 7:00am - 3:00pm

• Provide compassionate hands-on nursing care to facility residents
• Administer medications and treatments as ordered
• Monitor residents conditions and report changes appropriately
• Complete accurate and timely documentation
• Assist with activities of daily living and other patient-care needs
• Communicate effectively with residents families and facility staff
• Follow all nursing standards policies and procedures

• Current Illinois LPN license
• Reliable transportation to the facility
• Strong communication and clinical skills
• Dependable compassionate and patient-focused
• Ability to work independently while also being part of a team
• Previous nursing facility or long-term care experience is a plus
