Position Title: LPN
Location: Groton NY 13073
Duration: 13 weeks
Work Schedule: Variable evenings
Pay rate: $45- $55/hr.
Responsibilities for this position include but are not limited to:
- Provide direct nursing care to residents in a Long-Term Care (LTC) / Nursing Home setting.
- Administer medications and treatments as ordered.
- Perform and document resident assessments and monitor vital signs.
- Monitor residents for changes in condition and promptly report changes to physicians and the healthcare team.
- Perform wound care and dressing changes as needed.
- Assist residents with activities of daily living (ADLs) and provide compassionate patient-centered care.
- Develop implement and update resident care plans as needed.
- Participate in resident care conferences and coordinate care with CNAs nursing staff physicians and other healthcare professionals.
- Provide direction and supervision to CNAs and other nursing staff as assigned.
- Complete accurate and timely clinical documentation using PointClickCare (PCC).
- Follow infection control procedures patient safety standards and facility policies.
- Requirements:
- Active New York LPN License required.
- Minimum 1 year of Long-Term Care (LTC) experience required.
- BLS Certification required.
- Experience with PointClickCare (PCC) preferred/required based on facility workflow.
- Strong knowledge of medication administration patient assessment wound care and care planning.
- Previous Charge Nurse experience preferred.
- Must be comfortable working Evening Shift: 2:00 PM10:00 PM 5 days per week.
- Must be available to work every other weekend.
- Must be comfortable working approximately 40 hours per week.
- OT may be available when there are call-offs.
