Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and detail-oriented Long-Term Care Functional Screener to join our GROWING IRIS Consulting Team in Milwaukee WI!
In this role youll help individuals enrolled in the IRIS Program (Include Respect I Self-Direct) determine their eligibility for publicly funded long-term care services. Youll be part of a mission-driven team that empowers people to live independently and meaningfully.
- Build trusting relationships with IRIS participants
- Administer Long-Term Care Functional Screens (LTC FS) annually or as needed
- Coordinate and document LTC FS renewals
- Provide guidance to help participants maintain funding eligibility
- Collaborate with ICA team members and community partners
- Participate in staff meetings and multidisciplinary conferences
- Prepare reports and may testify in hearings or reviews
- Ensure compliance with federal and state documentation standards

- Service Awards & Recognition
- Bachelors degree in social work psychology human services nursing special education or related field
- Minimum 1 year of experience working with seniors or individuals with disabilities
- Valid LTC FS Certification (or willingness to obtain)
- Bilingual in Spanish Hmong Arabic Russian or other languages is a plus!
- Strong commitment to participant autonomy and meaningful living
- Comfortable with smartphones tablets and computer systems
- Knowledge of human behavior social dynamics and community resources
- Strong organizational and planning skills
- Ability to work independently and collaboratively
- Valid drivers license & reliable vehicle
- Satisfactory driving record (MVR check required)
- Must pass Caregiver Background Check
- Willingness to travel frequently (day trips occasional overnight)
