Job Summary

The Licensed Practical Nurse (LPN) delivers high-quality direct patient care under the general supervision of a Registered Nurse (RN). This role serves as a primary clinical channel of communication between the patient and the care team executing treatments monitoring outcomes and ensuring strict adherence to infection control policies.

Key Responsibilities

• Provide comprehensive direct nursing care including managing personal hygiene and assisting in formulating and updating patient-centered care plans.
• Administer prescribed medications and clinical treatments including respiratory therapy modalities maintaining meticulous healthcare logs.
• Observe evaluate and rapidly reportshifts in patient health conditions or emergency indicators to the supervising RN.
• Instruct and educate patientsand their families during hospitalization stays and throughout the discharge protocol.
• Document clinical assessmentsand baseline activities using standard electronic medical records (PointClickCare).

Mandatory Requirements

• Education:Graduate of an approved accredited Licensed Practical Nurse program.
• Licensure:Active unrestricted Practical Nurse (LPN) license.
• Certifications:Active Basic Life Support (CPR) and Advanced Cardiovascular Life Support (ACLS) certifications.
• Technical Skills:Competency in computer documentation applications and clinical charting.
