JJAT Case Manager

The JJAT Case Manager coordinates services and planning for children and families that are referred and screened into the program. Their goal is to guide develop implement and monitor individual case plans and to ensure client and family voice is heard. This role will guide their team through the service process and phases and hold the central point of communication between the team and client. Services will be provided almost exclusively in the community: in families homes foster homes schools etc. The primary role is to ensure that all parties are working together toward stability for the child and family.

This is a hybrid position with a minimum of 2 days in-person with 1x day required on Wednesdays.

Key Ares of Responsibility:

  • Build plan facilitate and communicate meeting details with team members clients and families.
  • Maintain a caseload of 10 -15 clients.
  • Ensure to incorporate input and needs of the youth when forming the team.
  • Establish and sustain an effective team culture by inviting team members to propose discuss and accept ground rules for working together.
  • Over time work to transition facilitation to allow for youth and/or family members to be facilitator of their own meetings.
  • Cultivate and maintain relationships with community members and providers.
  • Utilize consensus-building techniques to meet the needs of the youth and family.
  • Provide assistance in case planning and consultation to team members on clinical issues as well as casework issues as appropriate.
  • Maintain team focus on scope of work for the progress/movement toward transition acknowledging and celebrate successes and transitions.
  • Stay abreast of all up-to-date Care Plans for each client and any other related documentation.

Essential Job Requirements:

  • Acceptance and ability to work with individuals and families with a variety of lifestyles behaviors cultural and spiritual practices with the ability to apply a social justice and equity lens to all aspects of this work.
  • Interest and ability in working with adolescents particularly those from marginalized groups.
  • Ability to work offsite at a community partner
