Hospice Case Manager (RN)

As an RN Case Manager youll have the opportunity to make a meaningful difference by helping patients and families navigate their care with confidence and support. This role combines hands-on nursing expertise with care coordination advocacy education and collaboration to ensure patients receive the right care at the right time.

RN Case Managers serve as a central connection between patients families nurses physicians social workers aides community agencies and other members of the interdisciplinary team. Youll help coordinate individualized plans of care while providing clinical guidance and supervision to LPNs aides and student nurses involved in the patients care.

Requirements: Active Indiana RN license or valid compact RN license

Preferred: Certifications/Licenses/Experience:

• Six (6) months of nursing experience
• Six (6) months of home care experience
• Case Management experience
