- Coordinate and process new client intake for the Michigan Home Help Program
- Collect and verify client eligibility documentation and authorization requirements
- Serve as the primary point of contact for clients families and referral sources during the intake process
- Communicate with Medicaid/state program representatives to facilitate approvals and authorizations
- Maintain accurate and up-to-date client records in the intake management system
- Collaborate with care coordinators schedulers and clinical staff to ensure seamless onboarding
- Follow up on pending cases and resolve intake-related issues in a timely manner
- Prior experience in homecare home health or healthcare intake coordination preferred
- Familiarity with the Michigan Home Help Program or Medicaid home care programs is a strong plus
- Strong organizational skills with the ability to manage multiple cases simultaneously
- Excellent verbal and written communication skills
- Proficiency in electronic health records (EHR) or intake management software
- Detail-oriented with a commitment to accuracy in documentation and compliance
- Compassionate patient-focused attitude with strong customer service skills
