Homecare Experienced PT

University Home Care Inc is seeking a dedicated and compassionate Homecare Experienced Physical Therapist (PT) to join our team in Livonia MI. Must have Kinnser this role you will provide high-quality physical therapy services to patients in their homes helping them regain mobility improve function and enhance their quality of life. You will work closely with patients families and other healthcare professionals to develop and implement customized care plans tailored to each patients needs.

• Conduct patient assessments and develop individualized treatment plans.
• Provide skilled physical therapy interventions in a homecare setting.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians nurses and other healthcare providers to coordinate patient care.
• Maintain accurate and timely documentation of patient care and treatment outcomes.
• Educate patients and their families on exercises safety and health maintenance.

• Valid Physical Therapist license in the state of Michigan.
• Previous experience in homecare physical therapy preferred.
• Strong clinical and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation to travel to patient homes in Southeast Mi Regional area.

• Competitive hourly wage of $70.00 - $80.00 per visit
