Location:Redlands CA - Hybrid. This is a field-based home health role where you will provide care in patients homes throughout the Redlands territory while completing documentation from the field.

Note:Candidates must have an active California Physical Therapist (PT) license. Home health experience 1 year of Physical Therapist experience a Masters or Doctorate degree in Physical Therapy BLS/CPR certification a valid drivers license reliable transportation HCHB/Homecare Homebase experience and OASIS experience are preferred.

Job Description

• Evaluate patients in their homes and assess mobility strength coordination pain and functional limitations.
• Develop individualized physical therapy treatment plans based on each patients needs and goals.
• Provide therapeutic exercises and interventions designed to improve mobility strength balance and independence.
• Educate patients and caregivers on exercises safety precautions and strategies for continued progress.
• Monitor patient outcomes and adjust treatment plans as appropriate.
• Collaborate with nurses physicians therapists and other members of the interdisciplinary care team.
• Maintain accurate and timely documentation of patient progress and treatment plans.
• Travel throughout the assigned Redlands territory to provide high-quality in-home care.

Qualifications

• Active California Physical Therapist (PT) license required.
• Home health Physical Therapist experience preferred.
• 1 year of Physical Therapist experience preferred.
• Masters or Doctorate degree in Physical Therapy preferred.
• Current BLS or hands-on CPR certification preferred.
• Valid drivers license and reliable insured vehicle preferred.
• HCHB/Homecare Homebase experience preferred.
• OASIS experience preferred.
