Health & Wellness Director Full-Time
At Sage Mountain Senior Living an Agemark Senior Living community were looking for a compassionate and experienced Health & Wellness Director to lead our clinical team and ensure exceptional care for every resident. As our Health & Wellness Director you will:
- Lead and support the wellness team while fostering a positive resident-centered culture.
- Oversee resident assessments care plans and clinical documentation.
- Partner with residents families physicians and community leaders to ensure outstanding care.
- Ensure compliance with state regulations and Agemark policies.
- Supervise medication management and nursing services.
- Hire coach train and develop wellness team members.
- Respond to resident concerns and emergencies with professionalism and compassion.
- Identify opportunities to improve resident health safety and quality of life.
- Serve as Manager on Duty as needed.
- Current RN or LPN license in California.
- Minimum one year of experience working with seniors.
- Two or more years of leadership or management experience.
- Strong clinical assessment and medication management knowledge.
- Excellent communication leadership and organizational skills.
- Ability to build relationships with residents families and team members.
- Comfortable using computer systems and electronic documentation.
- Flexibility to work occasional evenings weekends and on-call responsibilities as needed.
