Job Summary
STATEMENT OF THE JOBThe Geriatric Wellness Coordinator works as an integral member of Supportive Housing Programs for adults and seniors. The Geriatric Wellness Coordinator is responsible for providing a range of on-site wellness services that meet the behavioral health and social needs of residents. The Geriatric Wellness Coordinator will perform initial interview crisis intervention and ongoing therapy with individuals and groups. The Geriatric Counselor is responsible for meeting & assessing building tenants and developing & implementing appropriate and effective communal and individual wellness activities that support residents as they age in place. Under the supervision of the Program Supervisor the Geriatric Wellness Coordinator will provide in depth case management service along with supportive counseling and groups to residents. The goal is to support residents in maintaining permanent affordable housing. The Geriatric Wellness Coordinator will play an integral role ensuring that residents can age in place with dignity. The Geriatric Wellness Coordinator ensures that the Agency Mission is met and best practices are upheld in our work with the buildings residents. The Geriatric Wellness Coordinator reports directly to the Program Supervisor for day-to-day operations and social service AND RESPONSIBILITIESProvide supportive mental health counseling to tenants in need in the senior with assigned residents on a regular basis and discuss and document their progress with specific reference to their service plan goals. Provide groups to seniors around benefits entitlements wellness and psychoeducational to all referrals in a timely and professional manner and follow-up on referrals made on behalf of a consumer/family recognizing the need for effective collaboration to ensure the highest levels of service provision. Conduct and prepare intake assessments case logs documents case notes service plans for and maintain contacts with families social service support agencies and health residents for their needs and develops and implements support residents in developing meaningful service plan goals and provide support to assist residents in achieving their residents with arrears in obtaining OSD or financial management wellness goals and activities with the input of residents and other and promotes wellness the development of appropriate wellness educational and social programming is consistently taking care for targeted critical/high need resident Crisis Intervention and crisis follow-up through team approach in partnership with building property management and maintenance staff to identify and respond to residents in positive advocacy and helps resident self-advocate with community service providers when appropriate. Identify service gaps which impede Agencys ability to deliver services and function in an efficient and professional ongoing active outreach and creative engagement of contact and follow-up with service providers put in place for residents such as Adult Protective Services and/or outpatient treatment adhere to and enforce all the Agency policies regarding Confidentiality and Corporate Compliance regulations and maintain compliance with HIPAA residents to face to face meetings as requested by the staff meetings agency meetings workshops classes & in-service trainings on a regular basis to maintain current information on eligibility requirements for all basic other duties as FOR EDUCATION/CERTIFICATIONS/LICENSESBachelors degree in human services Sociology Psychology Social Work or related field required.
