- Evaluate patients and develop individualized treatment plans
- Provide speech language cognitive and swallowing therapy
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with nursing therapy and interdisciplinary teams
- Complete accurate and timely clinical documentation
- Educate patients families and caregivers
- Current Michigan SLP license
- ASHA certification (CCC-SLP) preferred
- Masters degree in Speech-Language Pathology
- Experience in SNF rehabilitation or geriatric care is preferred
- Strong communication organization and clinical skills
