Family Support Navigator

Company Description: NAMI San Diego & Imperial Counties (National Alliance on Mental Illness) is a nonprofit organization that provides free education support services and advocacy to improve the quality of life of everyone affected by mental health concerns.

Work location: This position is based in Imperial County. The work location is hybrid; that is our Family Support Navigator will work from their home office as well as at the office and in the community as needed. Given that our program serves individuals within all areas of the Imperial Valley our team may find themselves traveling to areas such as Ocotillo Salton City and Winterhaven.

Primary Responsibilities/Function: Our Family Support Navigator will support the Coordinated Family Supports (CFS) program where our team assists adults who are clients of the San Diego Regional Center in Imperial County as well as their families with connecting to services. Specifically they will support individuals and their families in pursuing their goals by providing case management assisting them with navigating and accessing services identifying and overcoming barriers planning education and other related services that will help individuals to remain successfully in their familys home.

Duties and Responsibilities:

• Maintain and manage an active caseload.
• Complete assessments to determine your clients needs; develop and provide a written plan for supporting the individuals in which you highlight needs and how these will be addressed.
• Maintain contact and meet regularly with clients.
• Connect individuals and their families with needed resources providing guidance proactively identifying and addressing potential barriers support with applying for needed benefits provide education as well as system navigation.
• Complete follow-up assessments to determine clients progress towards goals; document findings and update the clients plan.
• Document services intakes and visits within three business days.
• Attend and participate in individual supervision to receive support on goal progress validate if frequency of contacts is adequate and review documentation in support of ensuring it is complete and timely.
• Communicate and collaborate with staff from San Diego Regional Center our clients family members and other providers.
• Participate in meetings with the CFS team the San Diego Regional Center other providers and NAMI San Diego & Imperial Counties.
• Builds and maintains relationships conducts outreach activities with the community as well as providers/staff of the San Diego Regional Center and other providers. This may include giving formal presentations to large groups of individuals.
• Supports the Lead Family Supports Navigator with completion of reports and invoicing.
• Provide culturally and linguistically appropriate services.
• Works collaboratively in a teamwork style with demonstrated ability to relate effectively with people from diverse backgrounds.
• Other duties as assigned.

Accountability: Reports to the Lead Family Supports Navigator.

Minimum Requirements including linguistic and cultural skills education experiences licenses and certifications:

• Bilingual (English/Spanish).
• Associate-level degree in a human services field of study or3 years of experience in the developmental disabilities service delivery system; and
• The ability to communicate in the primary language of the client and their family; and
• Knowledge of the Regional Center system.
• Bachelors degree in a human services field of study (such as psychology social work child development etc.); OR an Associates degree in a human field of study and at least 3 years of experience supporting individuals living with intellectual/developmental disabilities (IDD).
• At least one year of experience supporting individuals living with intellectual/developmental disabilities.
• In-depth knowledge of Imperial Countys services resources and culture.
• Exceptional interpersonal skills positivity attention to detail flexibility and enjoy working in an ever-changing environment.
• Personal lived experience or with a family member or loved one with developmental disabilities and/or behavioral health challenges preferred
• Excellent planning organizing problem-solving time management and coordinating skills.
• Able to work with minimal direction and supervision.
• Demonstrated ability to maintain confidentiality.
• Strong positive viewpoint towards individuals with developmental disabilities mental health challenges and a history of substance use and their families.
• Natural ability to build and maintain relationships and find common ground with diverse stakeholders.
• Ability to get along and work effectively with others.
• Able to communicate effectively verbally and in writing.
• Regular predictable attendance is required.
• Completion of Medi-Cal Peer Support Specialist Training is preferred.
• Intermediate to advanced knowledge of computer software (Microsoft Office Word Excel) email and web navigation.
• Must be willing and able to drive your personal car possess valid California Drivers License and comprehensive automobile insurance as required by law. Must have a satisfactory driving record.
