Ascension Wisconsin is looking for a Family Medicine Nurse Practitionerfor our AMG Oak Creek Clinic!

• Join a collaborative team of five providers caring for patients of all ages.
• Schedule: Full Time; Clinic hours are 7am-5pm M-F. 2 Saturday mornings a month are required.
• Call structure: No call
• Practice setting: Outpatient
• Access to on-site lab
• Employed position within a nonprofit health system
• Obtain patient history and perform assessment via observation interview and examination. Order perform and interpret diagnostic studies. Perform preventative health assessments screening immunizations and patient care.
• Counsel and educate patients and their families/caregivers concerning preventative health treatment options and community resources.

Licensure / Certification / Registration:

Required Credential(s):

• Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or
  job transfer date.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
• One or more of the following:
  • Nurse Practitioner credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date.
  • Registered Nurse credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date.

Preferred Credential(s):

• BLS Provider. American Heart Association or American Red Cross accepted.
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration.

Education:

• Masters degree of Nursing required.
