This is an exceptional opportunity for a Nurse Practitioner or Physician Assistant to join a busy well-established outpatient rural primary care practice with a strong referral base and the full support of a robust health system network conveniently located on the campus of Ascension Calumet Hospital.

• Join an established team of 1 physician and 5 APPs
• Call structure: 1:7
• Practice in a high-volume rural primary care clinic with a supportive team-oriented environment within a busy and collaborative setting with an average of 12-15 patients per day
• On campus with Calumet Hospital which was recognized as a 2024 Top 100 Critical Access Hospital by the Chartis Center for Rural Health for outstanding performance among the nations rural hospitals.
• Provide comprehensive patient-centered care by conducting health assessments obtaining patient histories ordering and interpreting diagnostic tests diagnosing and treating acute and chronic conditions and delivering preventive care services.
• Educate and counsel patients and their families on preventive health treatment options and community resources while collaborating with physicians to develop implement and document individualized care plans.

Licensure / Certification / Registration:

• Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.

Education:

• Masters degree of Nursing required.
