Salary: $50.00 per hour - 20 hours per week (can supplement FT work w/ SOC and 60 day assessments)

• Lead and support the clinical team to deliver exceptional patient-centered care while overseeing all nursing operations and ensuring compliance with home health regulations and standards.
• Manage the creation and oversight of 485 plans of care review clinical documentation for accuracy completeness and regulatory compliance and monitor patient care plans and outcomes to ensure quality and effectiveness.
• Recruit hire onboard train and mentor Field Registered Nurses (RNs) providing ongoing coaching and performance support to foster clinical excellence.
• Develop implement and maintain clinical policies procedures and quality improvement initiatives to promote best practices and continuous agency improvement.
• Collaborate with physicians administrators and the interdisciplinary team to coordinate patient care resolve escalated patient or family concerns and support positive clinical outcomes.
• Prepare for surveys audits and regulatory reviews while helping shape the agencys clinical direction and supporting its continued growth and expansion.

• Active Registered Nurse license in the state of practice
• Strong leadership experience in home health hospice or a similar healthcare setting
• Excellent understanding of state and federal home health regulations
• Experience managing clinical teams and improving quality of care
• Strong communication organization and problem-solving skills
• Ability to mentor and support a diverse team
