Title:Community Outreach Specialist

Location:White Plains/Westchester County

Salary:Up to $100000

***Local travel within assigned territory.

Description:

The Community Outreach Specialistwill inpromote Always Compassionate Healths home care services within Westchester County and educate prospective clients and community partners about eligibility and enrollment processes.

Responsibilities:

• Conducting research to understand the local market trends demographics and competition toidentifyopportunities for growth

• Developing outreach plans and strategies tailored to the specific needs and preferences of the target audience

• Building andmaintainingrelationships with healthcare professionals community organizations senior centers hospitals and other referral sources to generate referrals and leads

• Traveling within the assigned region to meet with current and prospective clients as well as referral partners

• Organizing andparticipatingin community events health fairs and seminars to promote the home care services and educate the public about the benefits of home care

• Utilizing online channels such as social media email marketing and website optimization to reach and engage with potential clients and referral sources

• Building rapport with clients and their families addressing their questions and/or concerns

• Monitoring the effectiveness of outreach campaigns tracking leads and conversions and providing regular reports to management on key performance metrics

Requirements:

• Bachelors DegreeMastersdegree preferred

• Experience with enrollment/intake for clients in a home/community-based setting

• Demonstrated knowledge of entitlements; Medicaid DSS DOH rules and regulations/insurance verification processes

• Experience with educating clients on health insurance benefits entitlements andassistingwith the application/enrollment process

• History of developing andmaintainingrelationships with community partners and referral sources

• Access to personal transportation and willingness to travel within the assigned territory
