As Clinical Team Coordinator youll contribute to our success in the following ways:
- Well versed in hospice workflow and regulatory requirements to drive quality measures and proper documentation of eligibility.
- Expertise in EMR and Hospice platforms (SHP)
- Assures hospice regulatory compliance through review of hospice documentation.
- Processes hospice workflow including but not limited to admission discharge and HIS workflow.
- Provides feedback and coaching using standard Elara resources.
- Communicates effectively with all members of the interdisciplinary team.
- Monitors for trends when completing workflow and involves hospice leadership and education team to provide support to clinical team members.
- Promotes Elara Carings philosophy mission statement and administrative policies to ensure quality of care.
- Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
- Performs other duties/projects as assigned.
What is Required
- Associate or bachelors Degree and graduate of an accredited program of nursing.
- Current license as a Registered Nurse
- 5 years experience as a Registered Nurse in a Hospice environment
- Experience with Hospice quality activities is preferred.
- Must have dependable vehicle valid drivers license and current auto insurance in accordance with the laws of the state.
- Ability and willingness to travel approximately 10% to meet business needs.
You will report to the VP of Clinical Services.
