Job Summary
Case Manager
Neulife Rehabilitation is currently seeking a skilled and compassionate Case Manager to join our team in Mt Dora Florida. As a Case Manager you will play a vital role in providing support and guidance to individuals in need of rehabilitation services.
Responsibilities:
- Conduct assessments and evaluations to determine individual needs
- Develop and implement individualized treatment plans
- Coordinate and monitor the delivery of services to clients
- Collaborate with healthcare professionals and service providers
- Advocate for clients needs and rights
- Maintain accurate and up-to-date documentation
Requirements:
- Bachelors degree in social work psychology or related field
- Previous experience in case management or a related role
- Strong interpersonal and communication skills
- Knowledge of community resources and support services
- Ability to multitask and work in a fast-paced environment
