The Source is seeking a compassionate organized and mission-drivenCase Manager to walk alongside women and families by providing care coordination support and access to life-changing resources. What Youll Do

As a Case Manager you will:

• Conduct client needs assessments and develop individualized Care Plans
• Connect clients with community resources and government assistance programs
• Provide ongoing encouragement coaching and care coordination
• Conduct follow-up calls home and hospital visits
• Maintain accurate client documentation using Salesforce
• Lead and support client groups while collaborating with clinic staff to ensure excellent client care

What Were Looking For

Required Qualifications

• Committed Christian with a personal relationship with Jesus Christ
• Passion for serving women children and families while upholding the sanctity of human life
• Bachelors degree in a related social services field Registered Nurse (RN) or Certified Community Health Worker
• Strong organizational communication and computer skills
• Ability to manage confidential information with professionalism

Preferred Qualifications

• Case management or care coordination experience
• Experience in coaching mentoring or motivational interviewing
• Masters Degree in Social Work (Texas Licensed)
