Job Summary
The Case Management Extender coordinates post-acute care authorization requests for assigned payors ensuring timely submission and follow-up through determination. This role communicates authorization outcomes to case managers physicians and post-acute providers and documents all activity in the electronic medical record. The Extender facilitates peer-to-peer reviews supports denial and appeal processes in collaboration with the care team and families and monitors Bridge 2 Rehab cases to ensure efficient resolution and minimize financial liability.
Minimum Requirements
High School or GED required
Experience with Word Excel EPIC RightFax and ONBASE strongly preferred
