The ICMS Case Manager will work with clients to assess housing stability provide stabilization services and coordinate multiple services to meet each clients specific needs related to housing health mental health and Description Essential Duties and Responsibilities: Work with a diverse and marginalized client caseload of 20 clients experiencing many barriers including homelessness complex trauma medical mental health and substance that case management services to clients are reflective of the Whatever It Takes model which emphasizes customer choice harm reduction trauma-informed care and psycho-social that each client on caseload has an up-to-date and comprehensive biopsychosocial assessment and that this assessment is used in collaboration with the client to create individualized case management plans designed to improve quality of life and improved health with each client to develop creative goal plans aimed at improving overall well-being and housing stability; review and update quarterly and upon completion of and maintain a complete accurate and current client file with all required documents and data in agency records and electronic all client contacts in PIP format (Purpose Intervention Plan) and in accordance with Housing Department productivity with DHS and community partners to ensure clients are connected to primary health care and insurance to reduce need for emergency health care confidentiality of client files per HIPAA and all applicable for coordinating appointments transportation and follow-up services for clients accessing primary health care mental health care recovery services and other community clients in your vehicle to various agencies and healthcare providers in the greater Los Angeles area to increase community support services and community effective trusting relationships with clients with a focus on facilitating independence and maintenance of improved physical and mental in the screening assessment and enrollment of clients including orientation to program policies resources and a current thorough knowledge of community resources and utilize them to provide comprehensive wrap-around services to clients with becoming involved in daily activities scheduling and attending appointments budgeting socialization health maintenance sense of community and progress in cooperatively and cohesively with other clients of the staff team including participation in weekly staff meetings and staff Description Qualifications:High School Diploma or GED or EquivalentProficient Spanish Language skills to engage with monolingual Spanish-speaking clients in the same capacity as English-speaking clients. Must be able to substantiate language CA Drivers license reliable car auto insurance and an acceptable driving recordAble to transport clients in your vehicleBachelors degree preferred; or a minimum of two years case management experience working with individuals experiencing homelessness preferably those living with mental illness and/or substance addictions or an equivalent combination of education and experienceDemonstrated knowledge of case managementAble to work in a high-tolerance Harm Reduction model with clients who have multiple barriersSkill in non-violent crisis interventionComputer literate; able to effectively use a computerized database for client file management with basic skills in Microsoft Word Outlook and Excel programsDetail-oriented with strong time management organizational written verbal interpersonal and computer skillsAble to obtain and maintain CPR/1st Aid certificationMinimum 30 WPM typing speed preferredJob Description Work Environment:Combination of field and office required to sit stand bend; occasionally lift or carry up to 35 occasion walk or drive to different local necessitate working in busy and loud be exposed to elements like cold heat dust noise and need to bend stoop twist and sit throughout the day.
