Performs biopsycho-social assessments creates and updates individualized treatment plans provides individual family and group therapy for patients to Outpatient Behavioral Care Services. Specializes in work with adults and/or adolescents and families experiencing mental health/substance abuse problems.- Job Description
Essential Functions

Completes an accurate biopsychosocial assessment including competency in diagnostic determinations and determination of appropriate level of care for patient.
Provides direct and indirect clinical/therapeutic services to patients and their families with clinical supervision as needed.
Develops and individualized treatment plan including measurable treatment goals related to patients presenting problems. Participates as an active member of the multi-disciplinary treatment plan. -
Provides the most appropriate therapy specific to patients symptoms.
Performs any case management responsibilities involved with direct patient care. This includes but is not limited to Utilization Review Discharge Planning Family Involvement/Education timely and appropriate documentation of care provided communication with referral sources completion of short-term disability paperwork and communication with patients employers as well as coordination of care with multidisciplinary treatment team and outside community providers.
Participates in outcome monitoring activities.
Performs other duties as assigned.
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor manager director or executive.
Minimum Requirements
Education - Masters degree in Social Work Counseling Psychology or related human services field of study.
Experience - No previous experience required

Required Certifications Registrations Licenses
Currently licensed as a Licensed Master Social Worker (LMSW) in the state the team member is working or currently licensed as a Licensed Professional Counselor-Associate (LPC-A) in the state the team member is working Marriage and Family Therapists and Psycho-Educational Specialists or currently licensed as a Marriage and Family Therapist-Associate (LMFT-A) in the state the team member is working.
Knowledge Skills and Abilities
Group individual and family therapy skills
Psychiatric problem-solving abilities assessment skills treatment planning skills.
Communication skills
Interpersonal skills across multiple professions/disciplines
Knowledge of computer software and utilization of electronic medical record.
