  Essential Functions

  • Completes an accurate biopsychosocial assessment including competency in diagnostic determinations and determination of appropriate level of care for patient.

  • Provides direct and indirect clinical/therapeutic services to patients and their families with clinical supervision as needed.

  • Develops and individualized treatment plan including measurable treatment goals related to patients presenting problems. Participates as an active member of the multi-disciplinary treatment plan. -

  • Provides the most appropriate therapy specific to patients symptoms.

  • Performs any case management responsibilities involved with direct patient care. This includes but is not limited to Utilization Review Discharge Planning Family Involvement/Education timely and appropriate documentation of care provided communication with referral sources completion of short-term disability paperwork and communication with patients employers as well as coordination of care with multidisciplinary treatment team and outside community providers.

  • Participates in outcome monitoring activities.

  • Performs other duties as assigned.

  Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor manager director or executive.

  Minimum Requirements

  • Education - Masters degree in Social Work Counseling Psychology or related human services field of study.

  • Experience - No previous experience required

  Required Certifications Registrations Licenses

  • Currently licensed as a Licensed Master Social Worker (LMSW) in the state the team member is working or currently licensed as a Licensed Professional Counselor-Associate (LPC-A) in the state the team member is working Marriage and Family Therapists and Psycho-Educational Specialists or currently licensed as a Marriage and Family Therapist-Associate (LMFT-A) in the state the team member is working.

  Knowledge Skills and Abilities

  • Group individual and family therapy skills

  • Psychiatric problem-solving abilities assessment skills treatment planning skills.

  • Communication skills

  • Interpersonal skills across multiple professions/disciplines

  • Knowledge of computer software and utilization of electronic medical record.
