The Position:

Our client is seeking an experienced Nurse Practitioner to provide primary care services in assisted living facilities throughout the Albany region.

Part-Time Option

• 2 3 Full Days per Week
• Travel Between Hoosick Falls NY and Cambridge NY

OR

Full-Time Option

• 4 5 Full Days per Week
• Travel Between Hoosick Falls NY Cambridge NY and Troy NY

Requirements:

• Masters Degree in Nursing from an Accredited Nurse Practitioner Program
• Active New York State Nurse Practitioner License
• Active DEA Registration
• Currently Enrolled and Credentialed with Medicare and Medicaid (Required)
• Ability to Practice Independently (Minimum 3600 Clinical Practice Hours in Accordance with NYS Requirements)
• Primary Care Experience Required
• Strong Clinical and Communication Skills

Preferred Qualifications:

• Geriatric Experience
• Assisted Living Experience
• Long-Term Care Experience

Responsibilities:

• Monday Friday Schedule
• No Nights
• No Weekends
• Conduct comprehensive health assessments physical examinations and medication reviews
• Develop and implement individualized treatment plans
• Manage chronic diseases and acute medical conditions
• Coordinate care with facility staff specialists and family members
• Provide preventive care and health education
• Maintain accurate and timely clinical documentation
• Ensure compliance with Medicare Medicaid and regulatory guidelines
• Additional Duties as Required
