The Position:
Our client is seeking an experienced Nurse Practitioner to provide primary care services in assisted living facilities throughout the Albany region.
Part-Time Option
2 3 Full Days per Week
Travel Between Hoosick Falls NY and Cambridge NY
OR
Full-Time Option
4 5 Full Days per Week
Travel Between Hoosick Falls NY Cambridge NY and Troy NY

Requirements:
Masters Degree in Nursing from an Accredited Nurse Practitioner Program
Active New York State Nurse Practitioner License
Active DEA Registration
Currently Enrolled and Credentialed with Medicare and Medicaid (Required)
Ability to Practice Independently (Minimum 3600 Clinical Practice Hours in Accordance with NYS Requirements)
Primary Care Experience Required
Strong Clinical and Communication Skills
Preferred Qualifications:
Geriatric Experience
Assisted Living Experience
Long-Term Care Experience
Responsibilities:
Monday Friday Schedule
No Nights
No Weekends
Conduct comprehensive health assessments physical examinations and medication reviews
Develop and implement individualized treatment plans
Manage chronic diseases and acute medical conditions
Coordinate care with facility staff specialists and family members
Provide preventive care and health education
Maintain accurate and timely clinical documentation
Ensure compliance with Medicare Medicaid and regulatory guidelines
Additional Duties as Required
