Job Overview

The Integrated Care Nurse Practitioner provides personalized value-based medical care to patients living primarily with chronic kidney disease and end-stage renal disease. This field-based role includes in-home patient visits comprehensive health assessments care coordination preventive management and ongoing treatment planning.

The Nurse Practitioner will work closely with physicians field nurses community health workers primary care providers specialists and ancillary services to improve patient outcomes close gaps in care reduce avoidable hospitalizations and enhance patients quality of life.

This is not an office-based position. Candidates must be comfortable traveling throughout their assigned territory and should live in or near the Greater Seattle and Tacoma area.

Travel Requirements

Candidates must be comfortable traveling throughout the assigned territory for in-home patient visits.

The position may also require quarterly travel for up to three days to locations approximately 200 miles away. Approved hotel accommodations and rental-car expenses may be reimbursed in accordance with company policy. The hiring team will provide additional travel details during the interview process.

Key Responsibilities

  • Conduct comprehensive annual wellness examinations in patients homes and through virtual appointments
  • Perform health assessments and evaluate patients current health status
  • Develop implement and evaluate individualized treatment plans
  • Prescribe medications and order diagnostic testing as appropriate
  • Educate patients and families on disease management preventive care healthy lifestyle habits and available programs
  • Coordinate care with primary care providers specialists physicians and ancillary services
  • Identify and close gaps in care through preventive monitoring patient education and provider collaboration
  • Document complete patient histories examinations diagnoses medications allergies assessments and care plans
  • Complete all clinical documentation and required paperwork accurately and on time
  • Deliver evidence-based care designed to reduce avoidable hospitalizations and improve patient outcomes
  • Participate in integrated care team meetings
  • Connect patients with educational materials support programs and healthcare resources
  • Maintain established standards for quality compliance and patient satisfaction
  • Perform additional duties as assigned

Required Qualifications

  • Active and unrestricted Washington Registered Nurse and Nurse Practitioner licenses
  • Current Nurse Practitioner board certification through the ANCC or AANP
  • Current and unrestricted DEA certificate
  • Minimum of two years of direct patient care experience
  • Ability to practice autonomously without direct supervision
  • Valid drivers license reliable transportation and current automobile insurance
  • Ability and willingness to travel throughout the assigned territory
  • Proficiency with electronic medical records medical equipment information systems and point-of-care technology
  • Strong verbal and written communication skills
  • Knowledge of HIPAA OSHA and applicable federal and state healthcare regulations

Preferred Qualifications

  • Managed care Independent Practice Association or health plan experience
  • Experience conducting annual wellness visits or comprehensive assessments in patients homes or virtually
  • Experience working with chronic kidney disease end-stage renal disease or other complex chronic conditions
  • Experience in value-based care population health or care management
  • Bilingual Spanish skills
