We are looking for an experienced WMO Consultant to support residents who require social and practical this role you will manage the full support process from intake and assessment to implementation and evaluation of care and facilities.
You will work within a multidisciplinary access team alongside youth consultants case managers and support staff focusing mainly on household support mobility provisions transport solutions home adaptations guidance and day activities.
Key Responsibilities
Conduct intake conversations with residents via phone and home visits
Assess support needs and analyze complex situations
Advise on suitable care solutions facilities and support options
Identify risk situations and take appropriate action
Coordinate monitor and evaluate provided support and services
Prepare reports and maintain accurate case documentation
Handle requests related to household support guidance and day activities
Requirements
Completed HBO-level education
Several years of experience in a similar WMO-related role
Strong knowledge of WMO legislation and relevant regulations
Excellent communication and interviewing skills
Solution-oriented and decisive approach
Ability to quickly assess complex situations
Strong sense of responsibility and organizational skills
Good written reporting skills
